There seems to be a connection between insulin and reproductive hormones. It is not clear exactly how the two connect, but insulin levels increase seem to lead to higher levels of androgens. While people of both biological sexes have androgens, they are typically considered "male hormones". High androgen levels in women can lead to symptoms of PCOS and ovulation problems. Methformin and other insulin-sensitizing medications (such as rosiglitazone and pioglitazone) reduce excessive insulin levels in the body and can be used to treat PCOS. Metformin Use for PCOS There are several reasons why your doctor may prescribe metformin to treat PCOS, some of which are related to fertility. Insulin resistance Insulin resistance is common in people with PCOS. Methformin can be prescribed to treat insulin resistance, which could help regulate reproductive hormones and restart ovulation. Ovulation Induction Some research on metformin and PCOS has proven to be more regular and ovulation returns with treatment. In some cases, the effect occurs without the need for fertility medications like . However, some larger research studies did not find that taking metformin was beneficial. People with PCOS have a higher risk of developing OHSS. Some studies have indicated that metformin could reduce the risk of OHSS during IVF. However, it is not clear whether OHSS is reduced for other treatments. For example, research on gonadotropins alone (without IVF) found no difference in OHSS rates by adding metformin to the treatment protocol. Repeated miscarriage People with PCOS could be more likely to suffer abortions compared to the general population. Some studies have claimed that metformin could reduce the risk of abortion in patients with PCOS. However, many more studies did not find abortion reduction due to the use of metformin. For people who took metformin when they try to become pregnant, there has been some concern that stopping the medication once the pregnancy has been confirmed could increase the risk of abortion. However, studies have found that stopping the use of metformin does not increase the risk of abortion. Continue to take metformin during the first trimester of pregnancy does not seem to reduce the abortion rate. The safety of taking metformin during pregnancy is not well documented. Your doctor may want you to stop taking it as soon as a fetal heartbeat is detected. For weight loss PCOS is linked to obesity. Research has found that when metformin is compared to a placebo, the ovulation rate increased. The hope was that ovulation would lead to conception, which made metformin a possible option for the treatment of fertility. However, subsequent investigations were less hopeful. Although metformin alone can increase the chances of ovulation in some people, studies have not found a rise in pregnancy rates or live birth rates. The impulse of metformin ovulation may not necessarily result in pregnancy success. Treatment with Clomid, Letrozole, or one of these drugs in combination with metformin, is a better option for fertility. There are also free sales supplements that claim to increase fertility because they contain substances that are natural insulin regulators. An example is , a B-vitamin complex found in cantaloupe, brown rice and sesame seeds. When taken as a supplement, it does not usually cause gastrointestinal side effects that are common with metformin. Ask your doctor before starting any supplement. Although you might get OTC without a prescription, they can still interact with the medications you are taking. Side effects The most common side effect of metformin is stomach discomfort, usually diarrhea. vomiting and nausea may also occur. Taking metformin in the middle of a meal might help decrease these side effects. Side effects related to digestion of metformin may decrease over time. Some people find that food in particular triggers more stomach discomfort than others. The most serious side effects associated with metformin are liver dysfunction and a rare but severe side effect called lactic acidosis. While taking metformin, your doctor should monitor your kidney and liver functions. People with heart disease, liver, kidney, or lung disease should not take metformin.
